* The application form can be obtained from our school in person or downloaded from our school website. Please submit the application form in person or by Post. Documents Required as below:
- Photocopy of Birth Certificate
- Photocopy of Immunization Record (with the cover page)
- One photo (1.5 inch) attached to the Application From right hand corner
- Three $2.2- stamped self-addressed envelopes
(If you submit the application form by Post, need four stamped self-addressed envelopes) - Application fee: HK$40-
(If you submit the application form by Post, please pay by cheque and make your crossed cheque payable to “Tsuen Wan Trade Association Chu Cheong Kindergarten”.)
