* The application form can be obtained from our school in person or downloaded from our school website. Please submit the application form in person or by Post.Documents Required as below:
1. Photocopy of Birth Certificate
2. Photocopy of Immunization Record (with the cover page)
3. One photo (1.5 inch) attached to the Application From right hand corner
4. Three $2.2- stamped self-addressed envelopes
(If you submit the application form by Post, need four stamped self-addressed envelopes)
5. Application fee: HK$40-
